Why It Happens

Joint wear is not a sentence to sit still.

Somebody showed you an X-ray, said the word arthritis, and that was the end of the conversation. But cartilage wear is only one piece of why a joint aches. The muscles around it get weaker, so the joint carries load it was never meant to carry alone. The joint above or below stiffens up and quietly hands its work over. You move less because it hurts, and joints need motion the way roots need water. Old injuries and extra load sit on top of all of that.

Here is the honest part. The wear is the one thing we cannot undo, and it is rarely the only thing driving your pain. Strength, motion and how you load the joint can all change, and that is where the relief actually comes from. So the evaluation is cause-first: we work out which of those is costing you your mornings, then build the plan around it. Movement is part of the treatment here, not a reward you earn once the pain leaves.

It is rarely the first step, and it rarely changes what we do. Imaging can show wear in a joint, and plenty of people who have that wear have no pain at all, so the picture does not tell us what is driving yours. A hands-on evaluation does. If imaging turns out to be genuinely necessary, we help you get it. And in Texas you do not need a physician referral to see a physical therapist, so waiting on a scan only costs you time.

Yes, and movement is part of the treatment rather than a risk we tolerate. Joints do better with motion than with rest, and the muscles around a sore joint are what take load off it. Mild working soreness while you build strength is normal. Sharp pain is a signal to change the dose, not to quit. We pick the movement and the load for your joint one-on-one with a licensed physical therapist, every visit, and adjust it as you go.

It depends on which joints are involved and how long they have been like this, so you get a real answer at your evaluation instead of a number off a web page. Your plan runs through all four phases, Pain, Prime, Perform and Prevent, and you will always know which phase you are in and what is left. We are not going to promise to undo the wear in your joint. The goal is a plan with an endpoint: comfortable, stronger, and going home able to manage it yourself.
