The short version

This website collects what we need to call you back and nothing we do not. We do not sell it. If you want it gone, ask us and it is gone.

Here are the details, in plain English.

What the forms collect

Our appointment request form asks for your name, phone number, email address, and a short description of what hurts, how long it has been going on, and what it is keeping you from doing.

Our free benefits check form asks for a bit more, because your insurance company requires it: your name, phone number, email address, insurance provider, member ID, group number, date of birth, and ZIP code.

Every field exists because somebody at the clinic needs it to help you. If a question feels like too much to type into a website, leave it blank and call us at (432) 262-1001 instead.

Where it goes

Form submissions come to Cultivate Wellness & Sports Rehab and into the scheduling system we use to manage appointments and follow up with you.

We use that information to respond to you, confirm a time, check your benefits, and keep track of the conversation we are already having with you. That is it.

A website form is not a medical record. Tell us where it hurts and how long it has been going on, which is all the form asks for, and save the rest for your visit or a phone call.

Do not send diagnoses, imaging, test results, or anything else you would consider private health information through this site. We will collect what we need properly once you are a patient.

Cookies and analytics

This site uses cookies and basic analytics to see which pages people read and how they found us. It tells us that a page was visited, not who you are.

If you would rather not be counted, your browser settings can block or clear cookies. The site still works.

Text messages are opt-in

You only get texts from us if you check the box asking for them. Checking it is your choice, and leaving it unchecked does not change the care you get.

You can withdraw that consent any time. Reply STOP to a message, call the clinic, or email us and we will take you off the list.

What we do not do

We do not sell your information. We do not rent it, trade it, or hand it to anyone who wants to market something to you.

The only people who see what you send are the people who need it to take care of you.

Fixing or removing your information

Something wrong? Changed your mind? Email admin@cultivatept.com or call (432) 262-1001, Monday through Friday, 8:00am to 5:00pm, and tell us what to correct or delete.

You can also write to us at 2608 Loma Dr, Midland, TX 79705. A real person reads it either way.
